Personal Care offers the security of assistance with personal care and activities of
daily living while promoting the greatest level of independence possible.
Accommodations include single rooms with a half bath or suites. Personal Care
suites include a living area, kitchenette, bedroom and half bath. Private bathing
suites with whirlpool or shower provide an enjoyable and relaxing bathing experience.
Three delicious meals are included in addition to housekeeping services and 24 hour
staffing by a licensed nurse. A wide variety of recreational activities encourage
friendly social interaction and maximum independence.

| About Waiting Lists |
Often, residency in personal care is possible immediately or through
the use of our waiting lists. The Standard Waiting List which requires
completion of an application and a $30 non-refundable application fee.
This list serves those who are thinking of the future and expect no
immediate need for services. Applicants to this list may refuse
available openings as often as desired and they will retain their
position on the list.
For those interested in more immediate residency in personal care, the
Priority Waiting List is recommended. Applicants to the Priority List will
complete an application and pay a $2,000 deposit which is applied
toward the Entrance Fee upon residency. As openings occur, contact
is made in chronological order to those on the Priority Waiting List first,
then to the Standard List. Applicants may remain on either list in their
same order regardless of the number of previous openings offered.
Applicants to either list are subject to certain financial and medical
criteria which can be explained during an in-person interview. All
applicants are requested to carefully review all information about
Countryside and be familiar with policies related to the waiting lists
prior to application.
